These Terms govern

 

  • the use of this Application, and,
  • any other related Agreement or legal relationship with the Owner in a legally binding way.

The User must read this document carefully.

Consent of Sample Use for Quality Control and Health Outreach

To enable an effective evaluation of Mojo's services and confirm the results of your analysis, we may need to use the results obtained in the evaluation for system quality control. Residual semen may also be used to confirm the validity of machine performance, staff training, and improving Mojo's services. Data obtained in machine or service performance maybe used to demonstrate AI self-education, machine learning, or service provision and presented in in-house literature and scientific meetings. Mojo may process your personal data for the purpose of local clinical audit to assess testing and treatment outcomes as well as identifying product and service improvements. 

Mojo participates in medical research and may share data with ethically approved external research organisations. Mojo commits that no data beyond that required for research and as permitted by law will be shared.

Mojo may pro-actively contact you in the event that your results require attention.

 

Conforming to Instructions for Use

The use of the Mojo home-kit as intended in the instructions for use is necessary to ensure quality of the service and analysis provided by Mojo. Therefore, Mojo reserves the right to reject and not analyse a sample if the sample collection and shipping procedure is not followed. A compliant sample return process includes but is not limited to:

  • The kit QR code must be linked to the customer’s personal mymojo.ai account, to ensure sample identification;
  • The complete ejaculate is collected;
  • The pot is sealed with timestamp sticker, on which is written the sample production date and time;
  • The heat pack and cold pack are used as instructed;
  • The complete Mojo packaging is used to return the sample.
  • The sample was produced in accordance with the sample return booking schedule date.
  • The instructions for abstinence and sample production were followed as per the instructions provided.

 

If the kit is not usable, or the sample is rejected, Mojo can offer replacement parts.

 

The replacement part cost (which includes the material and shipping costs) will be at the customer’s charge in the following cases:

Instructions for kit usage not followed:

  • Sample produced too early (not on the day of sample return);
  • The sample return instructions were not followed, thus potentially impacting the results of the analysis.
  • Lid not sealed properly with the timestamp sticker;
  • Date and time of sample production not stated;
  • Sample returned with a cold cold pack instead of activated heat pack;
  • Heat pack or preserving liquid bottle misused, e.g. opened before date of sample return;
  • Part (preserving liquid, sample cup, heat ,pack, etc) misplaced by the customer;
  • Preserving liquid not kept refrigerated.
  • Incorrect delivery address provided by the customer when placing the order.

 

If the sample is produced before the day of sample return, it cannot be analysed. Therefore, if customer produces sample too early, or if they use the sample cup but are unable to proceed with the sample return on the scheduled date, Mojo recommends to:

  • contact us so that we send a replacement cup
  • recommend to discard sample and wait for a replacement to book a new return date to ensure a valid analysis

 

Cancellation and Refund Policies for Results Discussions, Consultations, and 3rd-party Laboratory Appointments

This policy applies to all services which are provided through online meeting appointments or with 3rd party services. This includes

  • Results Discussions
  • Medical Consultations
  • DNA Fragmentation Testing
  • Blood Testing
  1. Appointments must be cancelled or rescheduled 24 hours in advance. Mojo may accept cancellations or reschedules under the required time window at its own discretion.
  2. Any appointments that pass where no communication can be shown from the customer asking to cancel or reschedule, and the customer is not present for the appointment will be charged, and a new purchase will be required for additional appointments
  3. 3rd party providers of Mojo may have their own terms and conditions which must be read and agreed upon by the customer
